I run at 125-333 depending on what I'm doing. I have a simple toggle key to turn it to 125, 250 or 333. If I have to get a little more height I turn it up, then turn it back down for better game play. High FPS does cause some issues so be sure to turn it back down. Also, make sure that 'Sync Every Frame' is off in the main options or you are locked at 60 FPS.
Here is the conftg I created to do it for me:

Paste that into text file and save as fps.cfg in your cod/uo folder. While on a server, pull down the console and type: exec fps
Here is the same thing in a zip file. Unzip the cfg file into your call of duty/uo folder
